Many members of Plumbers and Steamfitters Local 434 have been on strike since Monday, after voting Saturday against a contract offer from an association of about 120 contractors in central and western Wisconsin. Union members will vote Saturday on a new offer from the Mechanical Contractors Association of Northwest Wisconsin. The improved offer was made during contract talks Wednesday and includes an overall cost increase of 4.7 percent the first year, 4.7 percent the second year and 4.6 percent the third year, said David Seitz, the associations executive vice president. A three-year contract that covered about 800 union plumbers expired Sunday, said Terry Hayden, business manager for the union local, which is based in Mosinee, Wis. In the final year of that contract, journeymen plumbers and pipefitters received $39.77 an hour in wages and fringe benefits, he said. Of that, fringe benefits represent about $11, he said.

Hypnotherapy is the therapeutic use of hypnosis. In hypnotherapy, hypnosis is used by psychotherapists to modify a patients behavior. According to the American Association of Professional Hypnotherapists, there is a 75-90% chance of effectively changing behavior with hypnotherapy. Once the patient has seen a hypnotherapist, self-hypnosis can be learned, and is sometimes recommended as part of the treatment plan. Self-hypnosis involves a patient using relaxation techniques and specific signals to clear his mind of extraneous thoughts and sensations.


Preparation


Hypnotherapy requires only that the patient desire to change a certain type of behavior. The hypnotherapist usually prepares for the sessions by asking the subject to stare at an object, suggesting, in a soothing voice, that the eyelids are becoming heavy, that the subject relax, and that he is becoming hypnotized. Then, the hypnotherapist conveys to the subject that it will be effortless to follow the hypnotherapists suggestions


Aftercare


Coming out of the hypnotic state is as simple as entering it. Waking from the hypnotic state slowly is preferable for optimal results. After hypnosis, subjects report changes in bodily sensations and describe an awareness of having gone into an altered state of mind.


Risks


Because hypnosis can sometimes completely remove or distract people from feeling pain, it is important that a doctor, or other appropriate medical specialist, assess the underlying medical or psychological condition. This assessment is important because, when using hypnosis to reduce pain or other physical symptoms, the pain may be alerting the patient to a problem that needs some other form of medical or psychological treatment. As an example, a brain tumor might be causing chronic headaches, and require immediate treatment.

Family doctors have begun using hypnosis to treat psychosomatic illness, control appetite, and reduce the need for medication. Because of the utility of using hypnosis to reduce the sensation of pain, it can make it possible for physicians to lower dosages of pain medication in cases of chronic illness. Because hypnosis is actually an intense state of concentration, physicians have now accepted the fact that patients can regulate their own heart rate, circulation, temperature, muscle tension, and other body functions, if necessary.

It is unlikely that you became a chiropractor to work in an ER. Imagine my surprise when members of our hospitals chiropractic department were asked to take a call in the emergency department (ED)! We are now in our second year of providing chiropractic coverage to the ED. In November 2000, Meadowlands Hospital Medical Center in Secaucus, New Jersey began offering chiropractic care in the ED. In an effort to satisfy the Joint Commission on Hospital Accreditations mandate to better address pain management, Meadowlands ED Director Gina Puglisi,MD, and Albert Cataffi,DC, former chiropractic department chief, developed and instituted a “chiropractor on-call” program. Dr. Puglisi readied the chiropractors with an orientation program to define the roles of the chiropractors and the rest of the ED in treating neck and back pain patients. Patients who present to the ED with neck or back pain are screened by the attending ED physician, who is responsible for ruling out serious pathology, fracture, neurological deficit, and other findings that might contraindicate spinal manipulation. The ED physician may order x-rays, blood work or other diagnostic tests. When a severe condition presents, orthopedists, neurologists or neurosurgeons are enlisted to take over the case. Historically, all patients without serious pathology were given prescriptions and discharged from the hospital, with or without adequate relief. With our chiropractor-on-call program, the ED physician now has the option of calling for a chiropractic consultation, which gives the patient the opportunity to receive additional relief.
The newly merged Michigan Association of Chiropractors (MAC) has awarded the Chiropractor of the Year award to two doctors - Drs. Christophe Dean and Thomas Klapp, who were honored for their outstanding commitment and leadership in the advancement of chiropractic in Michigan. The dual award marks the recent successful merger of the Michigan Chiropractic Association (MCA) and the Michigan Chiropractic Society (MCS). Dr. Dean, a graduate of Palmer College of Chiropractic, has been a chiropractor since 1975. He has served on the Michigan Chiropractic Council (MCC) board of directors and was elected MCCs president in 2004. Dr. Klapp graduated cum laude from Life Chiropractic College in 1979. He served as MCA president before the merger and now serves as MAC chairman. This is the third time he has been honored with the Chiropractor of the Year award.

Prosperity in the U.S. construction industry was again on the house, as the residential segment continues to exhibit most of the recent growth in the domestic building industry. The construction industry appears to be hovering at a level close to last years pace, says Robert A. Murray, vice president of economic affairs for McGraw-Hill Construction. Housing remains very healthy, and the upturn for non-residential building in February was a welcome development after a sluggish January. Murray says that higher steel prices could be putting a crimp in the nonresidential segments. Over the course of 2004, its expected that nonresidential building will see a modest strengthening trend. Admittedly, though, this years sharp increase in steel prices has made the nonresidential upturn less certain, given the impact that higher costs and materials shortages may have on individual construction projects. At this juncture, it appears that the nonresidential upturn will be dampened but not derailed, assuming steel prices settle back by midyear, but the situation clearly bears watching.

Eljers participation in the settlement is conditioned on finalization of an agreement between U.S. Brass, Eljer and the other parties to the litigation and confirmation of a plan of reorganization in the U.S. Brass bankruptcy. Rodent control is taking some interesting and not so encouraging turns. There is an initiative that will be introduced in California on the November 1998 ballot (assuming it gets the 500,000 signatures needed, which is a slam dunk in California). It is called an Initiative to Ban Cruel Traps. This initiative, in a nutshell, will make it illegal to use a body gripping trap. one that grips the mammals body or body part, including, but not limited to, steel-jawed leghold traps, padded-jaw leghold traps, Conibear traps, and snares. This initiative also states that, it is illegal for any person, including employees of the federal, state, county or municipal government, to use or authorize the use of any steel-jawed leghold trap, padded or otherwise, to capture any game mammal, fur-bearing mammal, non-game mammal, protected mammal, or any dog or cat.Now, stop and reread that paragraph and think about it for a minute. This initiative decides for us that we cant control agricultural fur-bearing pests any longer. For that matter, it specifically states that you cant trap anything that has fur on it for any reason, and that the U.S. Fish Wildlife Service, State Department of Fish Game and your local county ag commissioner or county trapper cant do it either. It does make provision for the protection of human health and safety (pretty considerate - you know how vicious those gopher and mole attacks on children have been lately), and it will allow you to use mouse traps in your basement. Oh, and by the way, it introduces two chemicals that can no longer by used - Compound 1080 and sodium cyanide. What are these used for? Well, its not rodent control. The state has been experimenting with these two chemicals for coyote control. They make a collar to put around a lambs neck. If the animal is unfortunate enough to be a victim of a coyote attack and the coyote punctures the collar its lights out for the sheep killer. The point here is precedent - get these two and then work your way on to household chemicals for insect and rodent control and sweep everything up in between.Since this is not a political editorial, all I can say is watch out for this one. Agriculture will lose big time, so tell a friend and lets make sure that we dont get tied down by bad law written by extremists. I think it is very important that we dont make our decision based on whats on TV. Hollywood is anti-trapping and they have a lot of money to spend to support their position. If you want a recent case history lesson you may look at the fine State of Colorado, which had a similar initiative pass last year by a 51% margin. After the smoke cleared, it was disclosed that Hollywood spent just a little over a quarter million dollars to promote the initiative. All the local state and county representatives, combined with the agriculture and fur trapping industries, could muster was $48,000, and they lost.

Pocket gophers are fossorial (burrowing) rodents–so named because they have fur-lined pouches outside the mouth, one on each side of the face. These pockets, which are capable of being fumed inside out, are used for carrying food. Pocket gophers are powerfully built in the forequarters and have a short neck; the head is fairly small and flattened. The forepaws are large-clawed and the lips close behind large incisors. This feature allows them to use their incisors for digging without getting a mouthful of dirt, a unique feature to gophers. There are over 30 species of pocket gophers in the Western Hemisphere. Pocket gophers vary in size from 5 1/2 inches in length to over 13 inches in length. Litter sizes vary from one to 10 but typically average three to four. In the northern part of their range they have one litter per year, as opposed to up to two in the southern ranges. Young gophers are dispersed above ground and are capable of swimming as well on the search for suitable habitat to begin new burrow systems. Pocket gophers feed in three ways: they feed on roots, above-ground vegetation, and they pull vegetation into their tunnel to be stored in food caches for later consumption. In the vineyard, pocket gophers girdle vines, damage roots systems (most often in new plantings), and cause multiple-irrigation problems by promoting erosion and damaging drip lines. They are solitary little beasts, vigorously maintaining and defending their tunnel system. Depending on the species and available habitat, gopher population density may vary from six to eight per acre, to up to 62 per acre. Gophers do not hibernate and only cohabitate during the breeding season The burrowing activity of one gopher produces up to 70 mounds in one month. So how do you control such an active but solitary rodent? The fact that a gopher is solitary works to your advantage. You have three basic pest management tools for gopher control: Biological control, chemical control and trapping.

QUESTION: Recently, administrators informed us that OR staff members must wear name tags, and they have provided us with lanyards to wear attached to the name tags. What is AORNs position on wearing a name tag in the OR? If a name tag is required, can we wear the lanyards around our necks, or are these considered in the same category as necklaces? Like a necklace, a name tag could be a potential source of contamination if the lanyard is dangling around the neck of perioperative personnel during prepping or when the person is near the sterile field. During this time, the name tag should be secured in a pocket or under a snapped warm-up jacket to prevent it from dangling or falling into the surgical area and leading to possible contamination. An alternative method of attaching the name tag (eg, a pin, a clip) may decrease the risk of the name tag contaminating the sterile field or a prepped area. The Methodist church has set up a company to manage an ethically correct portfolio. It joins the Co-operative Bank and a small, but growing number of organisations that believe they can influence the way corporate Britain behaves by withholding their money from companies involved in alcohol, tobacco, armaments, gambling and pornography. They call it “ethical” investing. Say you are buying stock in Hanson, one of the biggest cigarette manufacturers in the country (it owns what used to be called Imperial Tobacco, maker of Players and Embassy). Its stock market value is about pounds 10bn. To have any significant effect on the company, you would have to pick up at least half a per cent of the company - that would cost you pounds 50m. The Methodists may well have access to funds on this scale. Presumably, they wont be buying Hanson shares. But would it not be making a stronger ethical statement if it did buy a slug of Hanson and started fighting from within? The Methodists would be one of the dozen biggest shareholders, and Hanson, famous for working “in its shareholders interests”, would have to sit up and listen to them rather than dismissing them as a bunch of ignorant do-gooders.
